Do ALL of Voldemort's followers get snazzy Dark Marks, or just his most hardcore minions? I mean, back in the first war, when the members of the original Order of the Phoenix were all scared and dying off and suspecting each other, wouldn't it have been easier for Dumbledore to just make everyone roll up their sleeves and check for spies? Or did Peter not have a Dark Mark?

Peter didn't have one because he got one in GoF, and he was a spy.
But Sirius didn't seem to know about them in GoF. I know musesfool has written a post about it. (A while ago, now, though.)
There seems to be a fanon conception that Bellatrix is the only femal DE, so Narcissa wouldn't have one, (and no one's saying she's a DE) but it wouldn't always signify who you could trust.

Maybe Deatheaters could use their dark magic skills to cast a really good glamour spell and cover up their Marks? I dunno, I can't get my brain around so many things that don't make in JKR's world.
